Clay Workshop for Teens in Grades 6th – 12th

I,  the above named parent or legal guardian of the above name student(s) , hereby give permission for my child(ren) to participate in the art, drawing, and handcraft activities organized at this event. I acknowledge that these activities involve materials and tools that may pose risks, including but not limited to:

• Allergic reactions to art supplies
• Stains or damage to clothing
• Accidental injuries from misuse of materials

I accept full responsibility for any associated risks.

Waiver of Liability: In consideration of my child’s participation, I waive, release, and discharge the event organizer from any claims arising out of injury, loss, or damage occurring during or in connection with the event.

Medical Authorization: In the event of a medical emergency, I authorize the event organizer to obtain emergency medical treatment for my child if I cannot be reached. I agree to bear any costs incurred.
